The level of care your loved one requires depends on the nature of their illness and how well their symptoms are managed. Our interdisciplinary team continuously assesses each patient’s condition to ensure they receive the most appropriate level of hospice care at every stage of their serious illness.

Routine Hospice in Home Services

When a patient is in the final stage of their illness and chooses to remain at home, routine home care is often the most appropriate level of hospice service. Patients at this level typically experience pain, nausea, vomiting, and other symptoms that can be managed through regular visits from our registered nurses, hospice nurses, hospice aides, and other members of the clinical staff.

Our doctors, attending doctors, skilled nurses, hospice nurses, and licensed staff coordinate with the patient’s physician to develop an individualized plan of care tailored to that person’s specific needs. Because the disease process is rarely linear, our clinical staff adjusts scheduled visits as needed — ensuring every patient receives the right level of attention as their condition evolves over the normal course of their illness.

Continuous Hospice in Home Services

When a patient’s symptoms become unpredictable or acute — particularly during a period of medical crisis — we may recommend continuous care. This level of service provides round-the-clock medical supervision in the home, with at least one registered nurse and additional trained nurses on call, present at all times to provide care, administer treatments, and manage acute symptoms as they arise.

The goal of continuous care is to reduce pain and bring stability to the most severe medical conditions so that the patient can transition back to routine care at home. When this is not possible, our clinicians will focus every available resource on keeping your loved one as comfortable as possible. Throughout this process, our social workers remain available to provide emotional support and practical guidance to families navigating this profound transition.
